To help us deliver the best care possible, please bring the following items to your visit: 

  • Your insurance information
  • Your primary care practitioner’s contact information
  • Your preferred pharmacy number
  • A list of your current medications, including over-the-counter medications and supplements
  • A list of medications you have tried in the past, including doses, duration and side effects you may have encountered
  • The results of any tests you have had relating to your health concern. This may include CT scans or MRIs (please bring discs and reports), bloodwork and evaluation reports by other consulting physicians
