About The Collaborative Care Center at Weill Cornell Medicine's Department of Psychiatry
The Collaborative Care Center (CCC) was established in 2016 to serve the mental health needs of individuals who receive their Primary Care at Weill Cornell Medicine. The CCC provides time-limited (short term) treatment for people struggling with many types of psychological distress, including issues related to mood and anxiety, as well as the more general impact of life stressors. Our services include expert consultation, evidence-based short-term psychotherapy services (including Cognitive Behavioral Therapy), and general psychiatric care (including pharmacotherapy).
For New Patients:
To receive a consultation at the CCC, you need to already be an established patient in Weill Cornell Primary Care Medicine. First discuss your concerns with your Primary Care Provider. If you and your PCP decide that a consultation with the CCC is needed, they will place a referral to the clinic.
After the referral is placed, you will receive a phone call from the CCC staff within a few days. They will either schedule you for an initial consultation with one of our clinicians, or connect you with our Continuity of Care Coordinator to explore other resources if that is more appropriate. Please note that at this time, all of our appointments are video visits through the HIPPA-protected Weill Cornell Connect patient portal.
Prior to your first appointment, you will receive a set of questionnaires via Weill Cornell Connect. These are essential for us to get to know you and to better understand how to help you. Please take the time to fill them out before your scheduled appointment.
For Existing Patients:
If you are currently in treatment at the CCC, the best way to communicate with us is through the Weill Cornell Connect patient portal. You can access Weill Cornell Connect using the app or on the web to send messages to your clinicians, request appointments, or request medication refills here.
For any non-urgent needs, you can also reach us by phone at: 646-962-6946.
For urgent requests, the CCC’s answering service can be reached by dialing (212) 517-1699 and stating your clinician’s name.
