• Primary care practitioner’s contact information
• Preferred pharmacy number
• List of current medications, including over-the-counter medications and supplements
• List of medications tried in the past, including doses, duration and side effects encountered
• Results of any tests relating to the health concern. This may include CT scans or MRIs (please bring discs and reports), bloodwork, tissue slides from prior biopsies, and evaluation reports by other consulting physicians
