Coronary Microvascular Dysfunction (CMD) is an underrecognized cause of angina and myocardial ischemia in patients without significant blockages in the major coronary arteries. Coronary microvascular dysfunction is a condition that affects the heart’s smallest blood vessels. These vessels are responsible for increasing blood flow to the heart when your body needs it—such as during exercise or stress. In CMD, the small vessels do not function properly, which can lead to symptoms such as chest discomfort, shortness of breath, fatigue, or reduced exercise tolerance, even when the major heart arteries appear normal. CMD is more common than once thought and is especially prevalent in women. CMD may be caused by a variety of factors such as high blood pressure, diabetes, high cholesterol, autoimmune conditions, smoking, and prolonged physical or emotional stress.
Coronary microvascular dysfunction can be diagnosed during a cardiac catheterization, a minimally invasive procedure allowing for direct evaluation of blood flow through the heart. During the procedure, a catheter is guided to the coronary arteries and contrast dye is injected. Even when the larger arteries look normal, specialized testing can be performed to measure how well the small blood vessels respond to medication and changes in blood flow. These advanced measurements help identify problems with the microcirculation that are not visible on standard imaging tests. The procedure is performed by our experienced interventional cardiologists and is designed to be safe, precise, and informative.
