Visitor Information


What to Bring

You should submit the following prior to your visit:

  • Copies of medical records of any cardiac-related tests or procedures, such as echocardiography, stress testing, catheterization films, CT/MRI scans, and/or EKG results.
  • Patients can also complete the  Medical Records Release Form and bring to their visit. Our office will request the records directly.

On the day of your appointment, please bring the following:

  • A list of all current medications (including names and dosages)
  • Preferred pharmacy information
  • Insurance card
  • Contact information of your referring physician and primary care physician
  • A list of any questions or concerns  

What to Expect

Please arrive 15 minutes early to your scheduled appointment and check in at the front desk. Our staff will verify your insurance information and collect any payment (i.e. copay).

Once in the exam room, our clinical technicians will take your vitals, including blood pressure and weight, and review your current medications.

Your doctor will evaluate your medical history, conduct a physical exam, analyze prior test results, and make any needed referrals (i.e. EKG). Our excellent team of clinicians and staff will ensure your visit is as comfortable as possible. 

Insurance and Billing

Our doctors accept most insurance plans. Our staff are experienced in this process and will be able to assist you with any questions on coverage and payment.

We welcome you to register for Weill Cornell Connect, our patient portal. You can use Connect to:

  • Manage appointments, both in-person or video visits
  • View lab results
  • Order medication refills
  • Pay or view a bill
